Report an Incident
Workplace incidents (injuries, illnesses, near miss, property damage) must be reported immediately. Reporting an incident helps to ensure that hazards and other contributing factors are corrected and future incidents prevented. Please complete the form below.
ENSURE CASUALTY RECEIVES FIRST AID, EITHER BY FIRST AIDER OR HEALTH CENTRE.
If the injury requires further medical attention, transportation must be provided to a Medical Facility by ambulance, taxi, or first aider.
YOU MUST CONTACT SECURITY SERVICES IMMEDIATELY.
If 911 has been called, contact Security Services immediately after so that first responders can be directed to the correct location.

ISOLATE THE AREA INVOLVED. DO NOT CLEAN UP THE AREA.
Building Services staff can be contacted to clean up blood and bodily fluids once Security or Safety and Health Services (204-632-2596) have released the scene.
DO NOT ALTER OR MOVE equipment or materials that were involved in an incident, unless it is necessary to free an injured or trapped person or to avoid creating additional hazards. Ensure equipment involved is locked out until Safety and Health Services have released the scene.
SERIOUS INCIDENTS must be reported to Manitoba Workplace Safety and Health Branch immediately at 204-957-SAFE (7233). Serious incidents include fatalities, injury from electrical contact, unconsciousness, fracture of skull, spine, pelvis, arm, leg, hand or foot, amputation, third degree burns, permanent or temporary loss of sight, a cut or laceration requiring medical treatment at a hospital, and asphyxiation or poisoning. Serious incidents include incidents involving the collapse or structural failure of a building, structure, crane, hoist, lift, temporary support system or excavation, an explosion, fire or flood; an uncontrolled spill or escape of a hazardous substance, or the failure of an atmosphere-supplying respirator.
BIOLOGICAL EXPOSURE INCIDENTS For incidents that involve sharps, splash to mucous membranes or any exposure to biological material, report to Health Services within two hours of exposure. If the incident occurred after hours, report to Security Services and proceed to the hospital for emergency care.
